A Heart for Sports
"Teen Screen America" ($2,500)
Funds will be used to conduct free community-based cardiac screenings for high school and collegiate student-athletes in an effort to prevent sudden cardiac trauma.

Families Forward
“Transitional Housing Program” ($2,500)
Funds will be used toward life skills, counseling, career coaching and child care expenses for low-income families in south Orange County who are at risk of becoming homeless.

Juvenile Diabetes Research Foundation
“Walk to Cure Diabetes” ($2,500)
These funds will help support the 2008 Orange County Walk to Cure Diabetes, which seeks to advance research for a cure for diabetes and its complications.

Cornerstone Schools of Washington
"Fit Cornerstone Kids" ($4,000)
The funding of this program will enable Cornerstone Schools to incorporate health science into their physical education program for at-risk Washington, DC, students.

Achievement Institute of Scientific Studies
($5,000)
This grant will provide funding toward college preparation and application fees for economically disadvantaged high school students from Santa Ana.

Boys & Girls Clubs of Anaheim
“Motel Kids Outreach Program” ($5,000)
This grant will enable at-risk homeless children sheltered in hotels and low-income housing access to services and youth development programs at the Boys & Girls Clubs of Anaheim, offering the hope of a positive future outside of poverty.

Giving Children Hope
“We've Got Your Back” ($5,000)
The funds for We’ve Got Your Back will support the continuation of a nutritional outreach program for elementary students in the Orange County school districts who have been identified as homeless.

Irvine Adult Day Health Services
“Adult Day Services Program” ($5,000)
Funds for this program will help support low-income seniors in managing and overcoming the effects of chronic health conditions that impact the activities of daily living and independence.

netz 4 Life, Inc.
“Leave No Rim Netless” ($5,000)
Funds will be used for the revitalization of neglected community athletic infrastructure to facilitate educational and leadership building programs for disadvantaged youth in Santa Ana.

Orange County High School of the Arts
“Camp OCHSA Community Outreach” ($5,000)
These funds will help the foundation offer a free 12-week session of classes to low-income Santa Ana children in grades 4 through 6 in dance, music, creative writing and visual arts.

Orange County Museum of Art
“School and Tour Programs” ($5,000)
This grant will help provide free high quality visual education programs within the Orange County school system to serve its target audience of Orange County students including Irvine 4th graders and underserved students from Title 1 schools.

Orange County Performing Arts Center
“Summer at the Center” ($5,000)
Funds will help support the continuation of Orange County Performing Arts Center’s 17-year history of providing a two-week program of musical / theater classes conducted by professional artists for at-risk high school students, in an effort to increase self esteem and discipline through active team participation in the arts.

Pacific Symphony
“Class Act Music Education” ($5,000)
Funding for "Class Act" will support the continuation of the Pacific Symphony flagship education program in its 14th year of introducing young people to symphonic music. Individual symphonic musicians are trained and placed in year-long residencies at 40 K-8 schools in Orange County.

Public Health Foundation Enterprises (PHFE)
“Children's Health Initiative of Orange County” ($5,000)
Funding for this program will help to provide access to basic health insurance and comprehensive health care services for the targeted audience of 62,000 uninsured children of Orange County.

South Coast Repertory
“Corporate Circle Education Fund” ($5,000)
This grant will help to support a variety of innovative and imaginative programs bringing professional theater to schools and neighborhood centers throughout Southern California reaching more then 68,000 students age six and above.

South County Senior Services
“Project Independence” ($5,000)
Funds donated to “Project Independence” will provide resources and improve the quality of life for the 2,000 Orange County seniors served by this organization by promoting health, wellness, nutritional well-being and socialization to enhance their independence.

Thanksgiving Point Institute
“Hands-on Discovery Field Trip Scholarships” ($5,000)
Funding will provide scholarships to reach 750 underserved students, enabling them to participate in field trips at Thanksgiving Point venues, including farm country, museum of ancient life and children’s discovery garden.

The Arc of Wake County
“Retirement, Leisure, Support and Services Program” ($5,000)
Funding of this program will help seniors with developmental disabilities access recreational activities that will increase their health and stamina, prevent aging-related illnesses and increase their connections to volunteers and members of their community, improving the senior’s quality of life.

The Lupus Foundation of America - Utah Chapter
“Walk for Lupus Now” ($5,000)
These funds will help support the 2009 Walk for Lupus Now, which seeks to support Lupus research, education programs and patient and family support services.

The Orange County Marathon
“Kids Run and Training Program” ($5,000)
Kids Run and Training Program is designed to assist pre-adolescent children in becoming more physically fit by promoting an active and healthy lifestyle, combat the onset of childhood obesity. This grant will help to increase the reach from 1,500 to 2,500 children.

THINK Together
“THINKing About Student Health” ($5,000)
This grant will help to implement the THINKing About Student Health program at eight Santa Ana middle schools serving approximately 1,800 low-income students who do not have access to out of school programs that promote physical activity. This program is designed to influence positive health choices and increase student awareness about the hazards of poor nutrition and physical inactivity.

Working Wardrobes
“Women's Day of Self Esteem” ($5,000)
This funding will support the 19th annual Women’s Day of Self Esteem, providing resources to approximately 125 at-risk women preparing to emerge from shelters and rehabilitation programs. This event will prepare them for interviews for new jobs, so they can become economically independent and regain their safety and health.

Vital Link
“Career Exploration Programs” ($5,000)
Funding will help to create a venue where more than 4,000 students in Orange County are able to gain valuable knowledge regarding a variety of careers spanning 20 different industry sectors, and make the connection to how they fit into the work world.

Volunteer Center Orange County
“Kid Healthy - Steps to Healthy Living” ($5,000)
Funding will be used to support this program in its 6th year, geared to promoting healthy eating and increased physical activity through an educational campaign and behavior modification program targeted at 4th and 5th grade students in six Southern California counties.

California Society for Biomedical Research
“Experimentation & Investigation: Societal-Based Issues” ($6,000)
This grant will support the development and publication of a curriculum resource piece for high school students and an instructional aid for teachers addressing a specific California Science Content Standard: Investigation and Experimentation within the sub-category, “Investigate a Science-Based Societal Issue.”

Make-A-Wish Foundation of Utah
“Wish Granting for Children with Cardiac Illnesses” ($8,200)
Funds will allow wishes to be granted for children in Utah between the ages of 2-1/2 and 17 with life-threatening cardiac illnesses, to enrich the human experience with hope, strength and joy.

American Red Cross
“Leaders for Life” ($10,000)
Funding will support the “Leaders for Life” blood donor programs for high school youth, which facilitates blood drives for patients in need in Orange, Ventura, Los Angeles and San Diego counties.

Camp del Corazon
“Summer Camp for Children with Heart Disease” ($10,000)
This grant will help to support Camp del Corazon’s annual residential summer camping programs, which are offered free-of-charge to children ages 7 – 17 with cardiovascular disease.

Goodwill of Orange County
“ATEC - Children's Services” ($10,000)
These funds will support the Assistive Technology Exchange Center, which provides tools and training to enable children with disabilities to communicate and learn basic skills such as reading, writing and math in order to prepare them for continued education and careers later in life.

Habitat for Humanity of Orange County
“Fullerton II” ($10,000)
Funding will support construction of four homes in phase II of Habitat’s Fullerton development by providing simple, decent, affordable homes for hard working, very low income families.

Laura's House
“Domestic Violence Emergency Shelter and Support Services” ($10,000)
Funding will help to provide shelter for 200 low-income abused women and their children, with emergency shelter and support through the Safety Net motel voucher program, 45 day emergency shelter and 90 day Step Ahead Transitional Living program.

Leonardo-Science
“Heart, Drugs and Youth” ($10,000)
These funds will support increased public awareness to teenagers and young adults on cardiovascular health, focusing on the effects of drugs and alcohol on the heart.

Orange County Council Boy Scouts of America
"Learning for Life At-Risk Promise Program" ($10,000)
These funds will support the Learning for Life, At-Risk Promise Program, which addresses the unhealthy behaviors of at-risk and disadvantaged youth in Orange County, by helping them to get back on track with their lives to reach positive goals, build self-esteem and learn about respect and responsibility.

Orange County United Way
"Community Impact Plan" ($10,000)
Funding will support a comprehensive plan that includes 59 programs that provide safety net services, 58 educational programs and 18 financial stability programs to address the needs of at-risk Orange County residents and move them from crisis to self sufficiency.

Project Tomorrow
"Science Docent Program" ($10,000)
These funds will be used to train high school students to become science docents by developing hands-on science lessons, which they will teach to elementary school students.

American Heart Association - Orange County
"29th Annual Orange County Heart Walk" ($15,000)
The American Heart Association will use this grant to help support the 29th Annual Orange County Heart Walk, which will help fund key cardiovascular research and educational programs.

Centro De Ayuda
"A Healthy Smile Will Bring a Healthy Heart" ($15,000)
This grant will support a new project in Puerto Rico’s rural communities to provide treatment of periodontal diseases in children and young adults.

Child Abuse Prevention Center
"In-Home Parent Empowerment" ($15,000)
Funds will help the “In-Home Parent Empowerment” program by providing parenting education services to families identified as being at-risk for child abuse and family violence. This program has shown success in the areas of increased communication skills, anger management techniques, appropriate discipline and knowledge regarding child development and safety needs.

Deborah Hospital Foundation
($15,000)
These funds will support the foundation’s objective to have an alternative for cardiovascular disease patients in Puerto Rico, who are in need of specialized diagnostic studies and open heart surgery.

Golden West College Foundation
"Expanded Nursing Program" ($15,000)
Golden West College will use the funding to support a weekend nursing program that utilizes college facilities as well as clinical studies in local area hospitals in Southern California. This program results in approximately 100 graduating nurses each year who will take their specialize program knowledge to a global community.

MIND Research Institute
"Orange County Math Initiative" ($15,000)
Funds for this program support a 5-year initiative that addresses the underachievement of 164 schools in 20 different school districts throughout Orange County, which score in the lowest three deciles of California’s Academic Performance Index.

“3rd Annual Cardiac Care Associate Symposium" ($2,000)
Funds will support evidence-based education of NPs, RNs, PAs and technicians in the cardiac care field.

Englewood Hospital & Medical Center
“CME Grand Rounds” ($3,225)
Grant to support presentation of “Improving Outcomes in Severe Sepsis & Septic Shock” by Emmanuel Rivers, MD, Henry Ford Hospital, Detroit, Michigan, and Institute of Medicine, National Academies.

California Pacific Medical Center Foundation
“Sutter Pacific Heart Center’s 3rd Annual Physician Symposium” ($5,000)
Funding to support symposium involving approximately 100 physicians with specialties in cardiology, internal medicine, primary care and family practice, addressing risk factors and recommended treatments for the female cardiac patient.

Marquette University
“Marquette Clinic for Women and Children” ($10,000)
Funding will support a new intensive cardiovascular case management program that will include coordinating the care of patients either being treated for or presenting risk factors for cardiovascular disease.

Mission Hospital Regional
“Heart Valve Screenings for Seniors" ($11,250)
This grant will support Mission Hospital’s heart valve screening program to seniors, which is provided at no cost as part of their community outreach program that promotes cardiovascular awareness and education.

Memorial Hermann Foundation
“Communication of Cardiac Risk to Underserved Women” ($12,500)
Funds will be used toward the development of an interactive tool to graphically communicate with minority, underserved and indigent women patients and to place risk into perspectives that are “patient friendly,” helping them to make more effective health changes compared to traditional delivery methods.

Andre Sobel River of Life
“Direct Family Assistance Program” ($15,000)
Funding will support a program in the Los Angeles area that will provide relief for urgent expenses to single parent families during a child’s catastrophic illness, enabling these caregivers to stay by their child’s side during this period of desperation.

Angel Heart International
“Mending Young Hearts in Gansu” ($15,000)
These funds will support under-privileged families with children born with lifethreatening congenital heart defects (CHD) in Gansu Province, China. The program provides for access to appropriate medical treatment for CHD, a support group toenhance communication and awareness and medical information from volunteer physicians, and also facilitates advances in treatment of CHD in Gansu.

CHOC Heart Institute
“Strategic Plan to Prevent Sudden Cardiac Death in Schools” ($15,000)
A grant to provide for a study that will lead to a white paper focused on causes and definitive steps that can be taken in schools throughout Orange County to address the issue of Sudden Cardiac Death that arise from exercise-related causes at school.

Good Samaritan Medical Ministry
“Summer Vietnam Mission” ($15,000)
Funding will support the annual clinic and patient sponsorship program involving healthcare professionals and volunteers that provide direct medical services to inhabitants of Vietnam via mobile clinics and regional and national hospitals.

Walter Sisulu Pediatric Cardiac Centre for Africa
($15,000)
These funds will be used to perform cardiac surgery on underprivileged children of Africa who have been diagnosed with correctible cardiac conditions. The Centre has provided successful cardiac intervention procedures to more than 200 children in Africa to date. Additionally, the program will be expanded to offer education, training and skills development to the medical community on the African continent.

Saddleback Memorial Foundation
“Heart Failure Disease Management Program” ($20,000)
Funding will support the continuation of this program, which began in November of 2006, and provides outpatient intervention and treatment to patients that suffer from congestive heart failure to enable patients to live full and active lives and reduce the number of readmissions resulting from lack of proper treatment and failure to make positive lifestyle changes.

Samaritan Health Foundation
“Heart and Vascular Outpatient Wellness Center” ($20,000)
A grant will enable this advanced practice, nurse-led heart failure outpatient center, started in October 2006, to continue to provide monitoring and education to cardiac patients to enable them to carry out self-care regimens and to ensure compliance with prescribed treatments. This program has successfully reduced the readmission rate at Good Samaritan Hospital.

Scott and White
“Heart Aware Reaching Rural Populations” ($20,000)
This grant is provided to develop a Texas-based pilot program that is ultimately aimed at reducing cardiovascular morbidity and mortality among the uninsured and underinsured individuals living in remote rural areas by delivering modern diagnostic, educational and treatment services through a mobile cardiovascular health clinic.

Share Our Selves
“SOS Chronic Care Program” ($20,000)
The funding provided to SOS will fund “bridge-care” services programs for the neediest patients in Orange County, the majority of whom are in need of cardiovascular diagnoses and treatments. SOS provides needed medical attention free of charge, including education, using a network of paid and volunteer physicians.

Children's HeartLink
“Training Minds, Saving Lives" ($25,000)
The grant supports this global effort to treat children with congenital heart disease and prevent the acquisition of heart disease in childhood from common illnesses such as rheumatic fever. Children’s HeartLink partners with 14 care sites around the world in developing regions.

Long Beach Memorial Hospital
“Heart Awareness and Primary Prevention in Your Community” ($25,000)
This grant will help to provide staffing and tools needed to expand the early cardiovascular screening and promotion of the awareness of cardiovascular health issues to underserved women in the Long Beach and north/east Orange County areas.

National Kidney Foundation NCA
“Cardiac-Kidney-Diabetes Connection” ($25,000)
Funding provides for the expansion of this Washington, DC, based initiative that provides free community health screenings and mobile health education for the at-risk and underserved population, which places a greater emphasis on the interrelationship between cardiovascular disease and chronic kidney disease.

Robert Wood Johnson University Hospital Foundation
"The RWJUH MIS Valve Replacement Surgery Training Program" ($25,000)
This grant provides funding for training of specially selected cardiac surgery teams from around the country in the area of minimally invasive surgery techniques in heart valve replacement. Training sessions are held twice a month and include five people per session.

St. Joseph Hospital
“From the Heart” ($25,000)
Funds will support the From the Heart program providing underserved, low-income Latino, Vietnamese, Arabic and Cambodian men and women with improved access to cardiovascular education, screening and treatment services through culturally and linguistically appropriate education.

The Mended Hearts, Inc.
“Mended Little Hearts” ($25,000)
The funding provides for supporting and expanding a network of 28 support groups and caregivers that currently exist in 17 states, which provides hope, care and support to families that have children with congenital heart defects and heart disease.

WomenHeart
“National Leadership Program” ($25,000)
Funds will aid in the ongoing training at the Mayo Clinic of a growing network of women patient volunteers from around the country to develop local outreach programs that promote heart disease education in their communities.

Society for Women's Health
"Stroke Symposium - What You Don't Know Could Kill You" ($35,000)
This grant supports a symposium that will address critical issues specific to stroke in women, including research, prevention, treatment, education and advocacy.

Providence St. Vincent Medical Center
($49,836)
This grant is provided to enlist a research fellow to mine the extensive database of the Medical Data Research Center to produce scientific publications and presentations for cardiologists, cardiac surgeons, nurses, technical health professionals and bioengineers throughout the world.

Brigham and Women's Hospital
"Team Heart: A Collaborative Treatment of Rheumatic Heart Disease" ($50,000)
Funds will be used for surgical repair or replacement of heart valves severely damaged by rheumatic heart disease (RHD) to needy patients in Rwanda. This 7-10 year program will help the Rwandan Ministry of Health to develop local capacity for cardiac surgery, reduce the numbers of patients with milder forms of RHD that progress to more severe stages and combat the source of rheumatic fever.

Conemaugh Health System
"Diabetes - Cardiovascular Disease Prevention Program" ($50,000)
Funding will enable a continuation of a program that provides education, screening and early detection of patients who are at risk for diabetes-related cardiovascular disease in the underserved communities of Western Pennsylvania. Education includes activities and lifestyle changes known to limit the progression of this disease.

FACTS - Care
"CVT Critical Care Training Center" ($50,000)
This grant will provide funding for accreditation of training courses and resources for cardiovascular-thoracic (CVT) critical care training to be provided for multi-disciplinary health care professionals at medical centers performing CVT procedures in the US and around the world.

Heart to Heart International
"Into the Heartland" ($50,000)
Funding will enable the further development of infant cardiac programs in the Volga and Siberian regions of Russia. The program includes examinations of nearly 500 children per site annually, resulting in 175-400 open heart surgeries. The program also supports the ongoing training of cardiac professionals at each location.

International Hospital for Children
"Project Guatemala - Healing Hearts" ($50,000)
This grant will provide for equipment and training to provide specialized pediatric care to children in Guatemala who might not otherwise receive this critical care. This is accomplished by sending surgical and diagnostic teams into the region, sponsoring surgical care for children at a network of partner hospitals and increasing the local capacity of partners to provide pediatric critical care in the local environment.

LSU Foundation
"GeauxHeart Baton Rouge" ($50,000)
The funding will support a series of health fairs to promote awareness, education and screenings for cardiovascular disease and to establish a satellite clinical testing facility in the underserved area of East Baton Rouge Parrish, Louisiana.

Massachusetts General Hospital
"Heart Awareness and Primary Prevention in Your Neighborhood" ($50,000)
Funds will be used to staff and provide needed resources for an expansion of a program that provides education and lifestyle training to low income women who are patients of Massachusetts General Hospital who are at risk for cardiovascular disease.

Stanford Biodesign Program
"Biodesign Innovation Program" ($50,000)
This grant will provide for the continuation of this fellowship program in its 8th year to provide graduate and postdoctoral training to physicians and engineers with the goal of developing the next generation of leaders in biomedical technology innovation.

University of Virginia
"Rheumatic Mitral Stenosis Humanitarian Collaboration" ($56,000)
Funding will provide for the creation and sustained delivery of cardiology screening clinics and therapeutic options for patients with rheumatic heart disease, as well as equipment and other resources to be used for the development of humanitarian education and clinical collaboration initiatives for the underserved population of the Dominican Republic.

Hoag Hospital Foundation
"Heart Valve Screening and Education for Orange County" ($60,000)
The Hoag Heart Valve Center will use these funds to provide free heart valve screenings and an educational program to the increasing number of senior citizens in nine targeted Orange County communities.

The HeartGift Foundation
"Mending Hearts" ($60,000)
Funding will allow this Texas-based organization to provide lifesaving open heart surgery to children from around the world with no access to cardiac care. Physicians, surgeons and cardiologists donate their time and more than 230 community volunteers provide thousands of hours of service to make this care available at no charge to these critically needy children.

International Children's Heart
"Pediatric Cardiac Training, Education and Clinical Services" ($75,000)
This grant will support the ongoing initiatives of developing pediatric cardiac services in the Dominican Republic and providing for surgical services to poor children of Afghanistan. The team travels to the Dominican Republic to perform surgeries, provide needed equipment and supplies and provide training to local physicians and nurses in pediatric cardiac surgery techniques.

Alliance for Aging Research
"Aortic Stenosis in Seniors: Patient and Physician Education" ($100,000)
These funds will enable the Alliance to conduct a patient study and provide educational materials on aortic stenosis, which is primarily caused by the calcification of the aortic valve. This will build on an earlier study performed in 2007 and materials will be made available through the Alliance’s website and highlighted in their online newsletter to more than 30,000 members.

St. Paul's Hospital Foundation
"Professorship in Structural Heart Disease" ($100,000)
St. Paul’s Hospital is a leader in advancing the treatment of cardiac valvular disease. The funding for this professorship will enhance existing resources and research programs at St. Paul’s and further develops this internationally recognized program with leading expertise in the management of valvular heart disease.

Surgeons of Hope Foundation
"From Managua to Managua" ($100,000)
The funds provide for two surgical teams to bring innovative cardiovascular surgical and medical care to in-need children in Nicaragua. The program includes direct care for approximately 30 children, and also includes training of local physicians at two state-of-theart hospitals in an effort to effect a long-term self-sustaining plan for ongoing education and administration of first-rate health care in this country.

The Larry King Cardiac Foundation
"Health Across America; Saving A Heart A Day" ($100,000)
Funds will support the program’s goal to Save a Heart a Day -- 365 in 2009. 165 lives were saved as a result of this program in 2007. The foundation supports life-saving treatment for individuals in the U.S. who, due to limited means or no insurance, would otherwise be unable to receive the cardiac-related surgeries and care they so desperately need. The LKCF partners with physicians and hospitals around the country to minimize the costs of these life-saving procedures.
